City take on the Buccaneers - the team McCarthy won the league with as a player seven years ago - at the Orlando Stadium on Wednesday night.
But McCarthy is unlikely to be involved as his wife has been hospitalised with a serious ailment, meaning he did not make the trip to Johannesburg with the rest of the Citizens squad earlier this week.
"Benni's wife is not well," City boss John Comitis told KickOff.
"She has been admitted to hospital with a collapsed lung.
"We understand his situation and our thoughts and prayers are with him and his family."
In McCarthy's absence, assistant coach Vasili Manousakis is expected to take charge as City try and hit back from their 4-1 defeat to Kaizer Chiefs last weekend.
Kick-off is at 19:30.
